ACPS understands that many individuals prefer to age well within a comfortable and familiar setting. Our professional consultants are trained specifically to aid the client with ageing well within their own homes.


ACPS places great importance on finding the most suitable care options to allow you to age well within their own homes.


What is In-Home Care?

In-Home Care is to assist those that wish to continue living independently within their own homes but may require assistance to do so.
There are Home Care Packages that provide tailored care services for your specific care needs to assist you with staying in your own home as you get older as well as provide you with the choices and flexibility you deserve.


Services that can be provided to assist you with living comfortably at home include:
-    Personal Care
o    Professional Personal Care Assistants to aid with bathing, dressing, toileting, general health care
-    Transportation Assistance (Escorts)
o     Assistance with transportation to attend appointments, go shopping or to attend social gatherings
-    Nursing Care
o    Professional Registered or Enrolled Nurses to assist with wound care, catheter management, diabetes management, medication management, PEG management, Post-hospital or Post-Operation Care
-    Domestic Assistance
o    Assistance with general household chores including cleaning, laundry, ironing
-    Home Maintenance
o    Minor general repair and care of your home and yard including changing of light bulbs, replacement of taps or lawn mowing
-    Home Modification
o    Installation of safety aids such as alarms, ramps and support rails in your home to match your care requirements
-    Food Support
o    Assistance with shopping for food, preparation and storing of food and/or delivering of meals to your home
-    Social Support
o    To provide companionship


There are two options to receiving home care
1.    Government Funded Home Care Packages (for those that are eligible)
There are 4 levels of Home Care Packages available
o    Level 1 – for those with basic care needs
o    Level 2 – for those with low care needs
o    Level 3 – for those with intermediate care needs
o    Level 4 – for those with high care needs.
Government Funded Home Care Packages generally have a waiting list and each package is funded differently according to the Australian Government and your selected service provider.


2.    Privately Paid Home Care Packages
For those that require immediate care services with no waiting list
For those that aren’t eligible for Government Funded Home Care Packages
Finding a Suitable In-Home Care Provider

There are numerous Home Care Providers and packages available and this may be overwhelming if you do not know where to begin looking
ACPS can assist you with finding the most suitable care package to allow you to age well within your own home.
Our placement process is personal and professional and will give you a better understanding of the Home and Community Care System in Australia. Your case manager will work with you to co-design a program to allow you to live an independent and active life.
After an initial conversation on the phone, our consultant will discuss and explain how this program can assist you and will then proceed with the following process if you deem this placement service to be suitable for you.


1.    Initial Meeting –
A personal case manager will be allocated to you and will be your point of contact for the whole process. An initial face-to-face 2 hour meeting will be organised for you and your family members to meet with your case manager.

Discussions will include:
-    The type of care required and the frequency you require these care services – Private or Government Funded
-    Your individual needs including care needs, spiritual needs, cultural and lifestyle needs
-    Financial considerations


Urgency of care needs – how long can you wait for these packages
For Government Funded Home Care Packages, there may be a waiting list involved.
For Privately Paid Packages, you will be able to assess immediate care assistance

The profess will ensure you understand the Home and Community Care system, any fees or charges required, the process of transitioning into receiving care as well as how to select the best package for you.

2.    ACAS Assessment -
If you have not already been assessed by an Aged Care Assessment Service (ACAS) team, ACPS can assist with arranging an ACAS team nearby your residential area to assess whether you are eligible to obtain a Home Care Package.

This will define whether you are eligible for Government Funded Home Care Packages or Privately Paid Packages,
If required your case manager can arrange to be present at the assessment

3.    Service Referral List –
Your case manager will shortlist and collate a small list of suitable Home Care Providers that can provide services to match your care needs according to your ACAS assessment. This referral list will be provided to you alongside our recommendations based on the information that you have provided us

4.    Shortlisting of care services–
Your case manager will assist with liaising with various Home Care Providers to determine the suitability of the care that they can provide alongside the waiting list time (if any) to receive these services.

Your case manager can arrange to be with you to meet your potential service provider to discuss your care needs and fees.

5.    Transitioning to services –
Once a decision as to which Service Provider and package you wish to receive has been made, your case manager can assist with the transition process to meet your new case manager as well as support with completion of all necessary paperwork.

Your case manager will ensure that the transition to this service is smooth and will support you throughout the whole process
