What is Residential Aged Care?

Residential Aged Care is regulated by the Australian Government for frail elderly individuals whose care needs prevent them from residing independently at home.


There are 2 care levels in Aged Care Homes – high care and low care.


To be eligible for a place within a Residential Aged Care, you will be required to undertake an Aged Care Assessment Service (ACAS) assessment to determine your care level and needs. You will then receive a letter to say that you are eligible for residential aged care.
Finding a Suitable Aged Care Home

There are numerous Residential Aged Care Homes available and you may not know where to begin looking. Each Residential Aged Care Home is different and has different care, services and activities to offer.


ACPS can assist you with finding the most suitable Residential Aged Care Home for you.


Our placement process is personal and professional and will give you a better understanding of the Aged Care System of Australia.
After an initial conversation on the phone, our consultant will discuss and explain how this program can assist you and will then proceed with the following tailored process if you deem this placement service to be suitable for you.


1.    Initial Meeting
A personal case manager will be allocated to you and will be your point of contact for the whole process. An initial face-to-face 2 hour meeting will be organised for you and your family members to meet with your case manager.

Discussion will include:
-    The level of care required based on your ACAS Assessment (see step 2)
-    Your individual needs including care needs, spiritual needs, cultural and lifestyle needs
-    Your location preference and dynamics of care provider services you are looking for
-    Financial considerations
The process will ensure you understand the Aged Care System, any fees or charges required, the process of placement and transitioning. Your case manager will address any concerns that you may have


2.    ACAS Assessment
If you have not already been assessed by an Aged Care Assessment Service (ACAS) team, ACPS can assist with arranging an ACAT nearby your residential area to assess the level of care that is required.
This will define the type of care that is required. If required, your case manager can arrange to be present at the assessment

3.    Service Referral List
Your case manager will shortlist and collate a small list of suitable Residential Care Facilities that match your requirements and needs according to your ACAS assessment.
This referral list will be provided to you alongside our recommendations based on the information you have provided to us.

4.    Viewing of Services
Your case manager will assist with liaising with various Residential Care Facilities to schedule appointments to inspect your interested options.
Your case manager can accompany you to these viewings if required.
We recommend you to keep notes on each facility including: your first impressions of the staff and the environment to allow you to make the right decision regarding services.

5.    Selection of Services
Once a decision as to which Residential Aged Care Facility you wish to be placed in is made, your case manager will assist with the admission process as well as the completion of all necessary paperwork.

Your case manager will ensure that the transition to this service is smooth and will support you throughout the whole process of moving in.
