ACPS understands that being a carer can be fulfilling but challenging and you may not always be available to provide ongoing care.
In these situations, ACPS can assist you with providing placement to short-term or long-term respite care in either an Aged Care Facility, Respite Care Centre or within your own home.
What is Respite Care?
Respite Care is short term or long term care in either a Residential Aged Care Facility, Respite Centre or within your own homes.
Respite Care is available to you when your carer is not able to be there.
Respite Care does not require an ACAT assessment and can be used up to 63 days each financial year.
We can assist with:
- Respite in the client’s homes
- Respite in day centres
- Respite in Aged Care Facilities
Finding Suitable Respite Care
There are numerous Respite Care options available and it may be challenging finding the most suitable one for your current care situation.
ACPS can assist you with finding the most suitable care package to allow you to age well and be independent while your primary carer takes a short break.
Our placement process is personal and professional and will give you a better understanding of the Respite Care System in Australia. Your case manager will work with you to co-design a program to allow you to live an independent and active life.
After an initial conversation on the phone, our consultant will discuss and explain how this program can assist you and will then proceed with the following process if you deem this placement service to be suitable for you.
1. Initial Meeting –
A personal case manager will be allocated to you and will be your point of contact for the whole process. An initial face-to-face 2 hour meeting will be organised for you and your family members to meet with your case manager.
Discussions will include:
- The type of care required and the duration you require these care services for
- Your individual needs including care needs, spiritual needs, cultural and lifestyle needs
- Your location preference and dynamics of care provider services you are looking for
- Financial considerations
- Options as to whether your prefer Residential Aged Care, Respite Homes or care within your own home.
- Insight into the care you current receive, who provides it etc.
The process will ensure you understand the Respite Care System, any fees or charges required, the process of transitioning into receiving care as well as how to select the best package for you.
2. Service Referral List – (if not receiving care in your own homes)
Your case manager will shortlist and collate a small list of suitable Residential Aged Care Homes or Respite Centres that can provide services to match your care needs.
This referral list will be provided to you alongside our recommendations based on the information that you have provide us.
3. Viewing of Services –
Your case manager will assist with liaising with various Residential Care Facilities and Respite Centres to schedule appointments to inspect your interested options.
Your case manager can accompany you to these viewings if required.
We recommend you to keep notes on each visit including: your first impression of the staff and the environment to allow you to make the right decision regarding services.
4. Selection of Services –
Once a decision as to where you wish to receive respite care is made, your case manager will assist with the admission process as well as the completion of all necessary paperwork,.
Your case manager will ensure that the transition to this service is smooth and will support you throughout the whole process of moving in.
Your case manager will also ensure smooth transition back to your primary carer when your respite care period has completed
This service is funded by the Commonwealth of Australia.
In some circumstances, there may be a small contribution fee for outings or any other special activities. Please discuss this with our Care Advisor.
